Form 1095-C

Form 1095-C for the 2025 tax year is available upon request. To request a copy of your form, please complete the 1095 Form Request for Plan Year 2025. Forms will be provided by either January 31, 2026, or within 30 days after the date the request is received. This notice will remain available through October 15, 2026. 

If you have trouble completing this online request form, you may email csandifer@richland2.org OR send a written request to Richland School District Two, Attn: Benefits Division, 124 Risdon Way, Columbia, SC 29223. The following information must be on the written request: First Name, Last Name, Work Location, Job Title, Email Address,  and Mailing Address.
